When Mrs. Will* found it hard to navigate her surroundings due to vision loss, Wesley Urban Ministries referred her to a Peer Connector who helped set her up with local activities.  Read our latest HSIIP Success Story to find out how the Connector program worked for Mrs. Will*.



The Hamilton Seniors Isolation Impact Plan (HSIIP) links seven organizations from across Greater Hamilton to reduce social isolation among seniors in the area. The HSIIP team will work together until 2019 to identify, reach, and connect isolated seniors. 

As one of the seven organiziations, the Gilbrea Centre will focus on researching the most effective interventions to decrease the social isolation of seniors and use an integrated knowledge exchange model to acheive our objectives.
